The utility of the diastolic stress test for the evaluation of exertional dyspnoea is a welcome addition to the HF guidelines. a–d | Resting Doppler echocardiography. Resting transmitral flow shows an early (E) diastolic filling velocity lower than the late (A) diastolic filling velocity and a ratio of E velocity and early diastolic velocity of the mitral valve annulus (E/e′) of 12. e–h | Stress Doppler echocardiography. After stress, E velocity and E/e′ increase, with an increase in tricuspid regurgitation (TR) velocity to 3.5 m/s, indicating a pulmonary artery systolic pressure >50 mmHg. Jessup, M. et al.
