Choose the following demographics

After completing each part, log into ASSIST to complete the Diagnostic
Log into ASSIST
Click on Diagnostics & Surveys Tab
Click “Start Diagnostic”
Click on drop-down “Choose a Template” and select Stakeholder Feedback Diagnostic
In Description, type Catholic Identity
Click on blue Start button

Choose Evaluative Criteria levels
Copy and paste answers to Diagnostic Questions
