Figure 4 | Proposed therapy scheme for anti-LGI1 encephalitis. Intravenous methylprednisolone is given for 3 days at 1000 mg daily or 5 days at 500 mg daily. Intravenous immunoglobulin is given for 5 days at 0.4 g/kg daily. Plasma exchange takes place over 3–6 sessions on alternate days. Rituximab is given for 4 weeks at 375 mg/m2 weekly. Cyclophosphamide is given for 6 months at 750 mg/m2 monthly. Azathioprine is usually administered as two daily doses of 75 mg, mycophenolic acid is administered as two daily doses of 720 mg or mycophenolate mofetil as two daily doses of 1 g. *Expect early recovery of seizures, but cognitive response can take longer.
