May vary greatly dependent on setting and organizational structure Inpatient: Ready for discharge planning Outpatient: Symptom improvement shown in assessment results Accomplishment of the client-defined goals Always reflect back onto their goals

Limitations to meet goals Expressed difficulties Progress in goal setting/accomplishing Areas of resistance, fear, barriers, and accomplishments Request clear instructions on risk assessments and the CM provider role
