MAPP Components Four Assessments Community Themes and Strengths Forces of Change Community Health Status Local Public Health System Assessment Community Themes and Strengths – identifies resources and issues within the community affecting health improvement Forces of Change – Identifies forces, such as legislation, policies, funding that are currently affecting a community and opportunities and threats associated with those forces Community Health Status Assessment – identifies priority health issues through a systemic collection and analysis of local, state, and national data on health factors Local Public Health System Assessment – a broad assessment of the different local public health system partners who work together to deliver PH services.
