Pre-contemplation Person is not considering change in their problem behavior. What is plan of care? Teaching plan? Engagement- Outreach, practical help, crisis intervention, develop alliance. How is this behavior effect you from reaching your dreams.

Contemplation Person considers that they have a problem and the feasibility and costs of changing that behavior, but are taking no steps to change. What is plan of care? Teaching plan? Persuasion- Education, set goals, build awareness of problem, identify supports

Preparation Person has made a decision to change but has yet to act on it. What is plan of care? Teaching plan? Persuasion- Education, set goals, build awareness of problem, identify supports

Action Person is actively taking steps to change, and normally continues for 3-6 months What is plan of care? Teaching plan? Action. Counseling, groups, support groups

Maintenance/Relapse Prevention
Person is trying to maintain a current level of change by preventing relapse. What is plan of care? Teaching plan? Maintenance- develop action plan to prevent relapse. Skill training,

Relapse Failure to continue action or maintenance stages begins another cycle. What is plan of care? Teaching plan?
