1 CAKE Q2 Total Awareness

2 Introduction Small marigins may be the difference between a minor and a serious incident. There can be many reasons for being unaware (thoughts being elsewhere, being tired or stressed, etc.) and this can affect how we react. Unawareness and human errors may result in incidents or accidents – hence we need to be TOTAL AWARE. ‘Remove the bag’:

3 Two main types of human errors:
Judgement -Thought it was safe -Thought I had enough time Memory - Did not remember procedure - Remembered the wrong procdure Perception -Did not hear alarm -Did not see signal -Did not perceive danger Action -Opened wrong valve -Pressed wrong button Unaware human errors: Human errors because of distracted choices: It is human to make mistakes However, it is also human to learn from own and others’ mistakes in order to achieve an injury-free workday Focus in this CAKE module is on errors because of personnel not being TOTALLY AWARE. Deliberate human errors: Human errors by intent; for example shortcuts, silent deviations etc. Not focus in this CAKE module

4 How to reduce the number of unaware human errors?
How can you reduce unaware human errors? Be totally aware of the work you are to perform Be aware of safe job performance Use your own expertise to select safety measures Be aware of the surroundings you are working in How can leaders reduce unaware human errors? Be in the field looking for potential hazards Point out specific conditions observed in the field and agree on how you follow up Observe and talk with executing personnel and give constructive feedback Judgement -Evaluate hazards and control them -Add safety margin Memory Use of checklists and procedures Perception -Be in control of your surroundings -Think of how to reduce the probability of failure Action -You follow what’s agreed -You stop if you are uncertain

5 Discussion: Which operations require special attention from you – on this hitch? What helps you to be present and attentive while working? What can cause you to be less attentive while working? What can you do in order to support others so that they remain attentive and avoid unaware human errors?

6 Suggestion: Include the CAKE topic in the daily safety focus. Examples of a daily focus that could be used would be: Possible distractions Routine work Risk Assessment Barriers Change in operation Pressurized equipment Enough rest Job safety
