Mission The Lincoln Trail District Health Department provides assurance of the public’s health and safety through assessment, intervention and preparedness.

July, 2013 Dear Fellow Community Member, Lincoln Trail District Health Department is pleased to offer you a look into your community’s health indicators. With this data, we are able to go out into our communities and work with one another to help prevent health issues and disparities. We also use this data to make policy changes as well as create programs that are individualized to each community. This is the ninth community report card for the Lincoln Trail District. As public health evolves, it is more important than ever to involve our community members in decisions that impact the community’s health and safety. A large part of this collaboration involves the use of data, such as the data contained in this report. We rate ourselves based on how close we are to achieving the goals established by Healthy Kentuckians 2020. We hope this report card is beneficial to your and your family. Our challenge continues to be to improve the health and safety of our communities through prevention and health promotion. We invite you to become a part of this challenge. Grading Criteria A Good B Fair (emphasis and planning needed)
C Poor (immediate action needed) Definitions: Age-adjusted rate – The hypothetical death rate that would have occurred if the observed age specific rates were present in a population whose age distribution were that of a standard population. Age-adjusted death rate Sum of (age specific death rate per 100,000 population x standard population in corresponding age group) (Sum of the standard population) Crude and age-adjusted death rate – The crude rate indicates the true current “risk of dying” in a population. However, since the crude death rate is influenced by the age composition of a population as well as by the number of deaths by age group, the crude rate is not appropriate for comparisons between different populations. The age-adjusted death rate is used to indicate the relative risk of dying among different populations by relating them to a standard population. By removing the influence of the age composition, the age-adjusted rate allows for valid comparisons between two populations.
