ACE = angiotensin-converting enzyme; BP = blood pressure; Ca = calcium; prn = as needed. * = Go to next stage if renal artery stenosis is a possibility, if serum potassium level is high, if advanced renal failure has occurred, or if pregnancy is being planned. If maximal dose is not tolerated, maintain lower dose and add next choice. ** = Use low-dose hydrochlorothiazide. Use furosemide when creatinine level is higher than 2 mg/dL or if nephrotic syndrome is present. If hyperlipidemia is a problem, indapamide could be used. *** = Do not use in patients with or thostatic hypotension or severe autonomic neuropathy.
