5 Engineering AFIs Largest number of EN AFIs were for:
Less than adequate design engineering product quality (18%) System engineering was main focus: Monitoring and trending (15%) System improvement plans (7%)

7 AFI Causes Largest single contributor to EN AFIs:
Ineffective engineering management (20%) Management fails to: establish or communicate standards and expectations hold engineers accountable monitor engineers’ work

8 AFIs Causes Largest problem area facing engineering:
Less than adequate engineering work management (19%) and Less than adequate work prioritization (14%) Prevents engineering and the site from getting the right work completed

9 AFIs Causes Another major contributor to AFIs is:
Less than adequate engineering infrastructure Inhibits engineer’s ability to do work Reduces effectiveness of engineering programs, processes, and procedures
