New Model Coordinated community based screening, assessment, and intervention services for children aged 18 – 26 months in the communities of Red Deer, Rocky Mountain House, and Olds

What are we doing? Screening – ASQ and ASQ/SE
- available at multiple locations 18 month immunization appointment Parent Link Centres Screening Days (Parent Link) Home Visiting Programs (Healthy Families, Infant Preschool Wellness) Early Intervention Program Pediatric Rehabilitation Program Native Friendship Centres Goal: 80% of 18 – 26 month olds screened once

Goal: Assessment occurs within 4 weeks of referral
Timely – Appropriate Professional Enhanced SLP, OT, PT FTE Created a psychologist position Goal: Assessment occurs within 4 weeks of referral

What’s unique? Multiple access to screening
Single point of entry/coordinated referrals - Intake Coordinator - Growing Together Coordinator Support for families - Child care - Transportation - At risk children Timely Assessments Growing Together Group - mild delay - hosted by Parent Link Centres - developed cooperatively by SLP, OT, PT, Psychologist and the Growing Together Coordinator (Social worker) - parents participate in both a learning session and a play based observation and practice session each week - lead by a trained paraprofessional (Growing Together facilitators) supervised by the professional staff - groups are 4 weekly sessions - role of the Growing Together Coordinator
