Figure 1 Exclusion and inclusion criteria. Transient was defined as less than 30 minutes. A positive serum marker assay (creatine kinase-MB or cardiac troponin) was based on the local institution’s diagnostic cut-off. Full-service hospitals were defined as hospitals with facilities for cardiac catheterization, percutaneous coronary intervention, and coronary artery bypass grafting.
