11 INITIAL CONCLUSIONS With the variation in definitions of the term “Bariatric” and the quite obvious variations in size of patients within this broad categorization it is immediately apparent that no single piece of equipment will meet the needs of all patients!

BREATHING: Patient may have difficulty breathing normally in certain positions.

PRESSURE ON ULCERATED AREAS: Depending upon the method of transfer being used extra care is required to avoid damage to ulcers etc.

15 DIGNITY OF TRANSFER With the likely number of carers required for non motorised movement of patients it is important to consider the personal dignity of each individual patient!

16 INTEGRITY OF PREMISES Depending upon the size (possibly unknown!) and number of Bariatric Patients there may be a need to reinforce floors and staircases.

19 NUMBER OF AVAILABLE CARERS
For a number of reasons, including moving & handling, transfer of patients and evacuation in an emergency, the availability of carers to assist such procedures may be of paramount importance. Detailed planning needs to be undertaken to ensure that all of these situations can be accommodated and conducted safely.

20 TRANSPORT OF BARIATRIC PATIENTS
London: Three bariatric vehicles and stretchers which can take patients up to 70 stone (444.5kg). For emergencies, it uses a third party provider. East Midlands: 225 bariatric capable vehicles for patients up to 50 stone (317.5kg). Six have hoists and lifts. East: Eight vehicles, with 16 bariatric stretchers, at a cost of £432,000 over three years.

21 TRANSPORT OF BARIATRIC PATIENTS
South Central: Has 10 bariatric vehicles arriving this year. South West: All ambulances have bariatric capability, including one 'Megasus trolley' for patients up to 60 stone (381kg). West Midlands: All 420 A&E ambulances are bariatric capable, but there are eight specialist vehicles and seven on the way, costing £114,000. Yorkshire: All ambulances are bariatric capable and the service has 109 bariatric stretchers.

23 NON CLINICAL ENVIRONMENT
The Regulatory Reform (Fire Safety) Order 2005 Summary: Responsibility to provide the means of safe egress lies with the operator (not necessarily the owner!) of the building. Evacuation Plans should NOT be reliant on the involvement of Fire & Rescue Services to work.

24 NON CLINICAL ENVIRONMENT
Equality Act 2010 Summary: People of ALL mental and physical capabilities are owed the same duty of care when it comes to the evacuation of a building. It therefore follows that to evacuate a 400Kg person in an emergency is of equal importance to anyone else of a lower weight!

30 CONCLUSIONS There are no exceptions when it comes to making provisions for larger people Careful planning is required in advance of occupancy of premises Consideration must be given to ALL areas of premises where larger persons may be present What do you do if the lift/elevator fails? Back-up plan essential!
