DROPPED OBJECT - Chisel loosened during test Synergicase: 15499, Date: Gina Krog Hook up – W 250 Description of Incident: The incident occurred during a test of a chisel machine in W250 module. During startup of chisel machine, the chisel loosened and bounced over the kick board and fell about 5 m. to underlying deck. Consequence: It was no personnel at underlying area (that was not cordoned off). The area is difficult to access. Vertical drop 5 m, fall energy 28.5 joules. Direct causes: (what went wrong): Chisel loosened due to weariness on the locking mechanism and defect pin. Root causes: Equipment shipped from the warehouse showing signs of extensive use / poor condition Insufficient initial inspection. Preventive measures / learning point • When testing tools this should only be done in a suitable area • Focus on visual check of the tool before and after work. • If in doubt about the tool is appropriate, contact the foreman. • Increased focus on initial inspection of equipment when arrival site . • Review of routine equipment check before sending to different sites Due to limited access to air, the equipment was tested in W 250 The chisel loosened from the locking mechanism.
