Background: sobue disease is benign focal amyotrophy of the distal upper limbs, often misdiagnosed as motor neuron disease. Routine magnetic resonance imaging (MRI) is often reported normal.which needs dynamic mri of neck. Materials and Methods: Patients presenting with insidious-onset hand wasting from December 2013 to october 2014 were evaluated electrophysiologically at annamalai university,Chidambaram.Cervical MRI in neutral position was done in 11 patients and flexion contrast imaging was done in 10 patients.

Results: All patients were 15-35 years of age
Results: All patients were years of age. Duration of illness was 3 months to 3 years. All (100%) had oblique amyotrophy. Neutral position MRI going to show 1.number of cases with loss of cervical lordosis, 2.number of cases with localized lower cervical cord atrophy, 3.number of cases with asymmetric cord flattening. 4.number of cases with intramedullary hyperintensity; 5.number of cases with flexion study showed loss of dural attachment, anterior displacement of dorsal dura, epidural flow voids and 6.number of cases with enhancing epidural crescent s.final results will be displayed at the time of presentation.

Conclusion: Findings are often present on neutral MR images and, in the appropriate clinical scenario, should prompt flexion MR imaging to evaluate anterior dural shift.
