2. Example of consequence of failure table

While much can be said about scoring the consequence of failure (see the Fundamentals of Asset Management training material on the USEPA website as referenced on the last slide of this Tool), in this example, a triple bottom line approach is presented. Note particularly the monetized consequences impact row highlighted in the slide. This row ties directly to the tolerable risk discussion in Step 1. What level of monetized community impact is the policy board willing to accept as the maximum level of consequence? Once that level is determined in dollars, then all failures, whether plant based or pipe, can be viewed from a level playing field.
