Important to consider Nursing homes a key part of the solution and are willing and able to do more 44% of fair deal applicants from hospital Nursing home also are step down units for those who need more support before going home and a step up for respite care Not all nursing home offer the same services. The good news: In ,930 patients were approved for funding for discharge to private and voluntary nursing homes under ‘Transitional Care Bed Initiative’ At December 2014 – 79.7% of the 719 persons delayed discharged were awaiting ‘long-term nursing care’ (573 people). At December 2017 – 57.9% of the 480 persons delayed discharged were awaiting ‘long-term nursing care’ (480 people). EMPOWERING MEMBERS LEADING & PROMOTING THE SECTOR INFLUENCING POLICY

What can be done: Development of acute care management in nursing homes e.g. IV Fluids A Geriatric Early warning score based on individualised baseline credentials Supporting early discharge and consultant outreach/ community supports Working groups in hospitals with MDT and community input and Stakeholder engagement EMPOWERING MEMBERS LEADING & PROMOTING THE SECTOR INFLUENCING POLICY
