Established in 1988 to provide TTUHSC student health services
Changed focus to provide primary care services to underserved populations in East Lubbock in 1998
A Nurse-managed FQHC that is a public entity
Co-Applicant Governing Board – Combest Health and Wellness Center Community Alliance (CHWCCA)
TTUHSC acts as fiscal unit
Administered by the School of Nursing for TTUHSC
All employees are hired by the SON

4. East Lubbock Promise Neighborhood
The East Lubbock Promise Neighborhood is a US Department of Education funded initiative committed to working with the residents of East Lubbock, Texas to support and advocate for our children – ensuring that both children and parents have access to the resources they need to grow, learn and succeed.

5. The Partnership Program Goals for the ELPN Health Component:
Increase the number and percent of children, from birth to kindergarten entry, who have a place where they usually go, other than an emergency room, when they are sick or in need of advice about their health
Utilized a Community Health Worker to:
Work exclusively with members of the East Lubbock community
Establish rapport between community members and stakeholders
Identify needs of the community
Offered extended clinic hours
Wednesday, Thursday and Friday’s open from 5:00 p.m. – 10:00 p.m.
Sunday’s open 1:00 p.m. – 6:00 p.m.

Increase the number and percent of children who participate in at least 60 minutes of moderate to vigorous physical activity daily
Increase the number and percent of children who consume five or more servings of fruits and vegetables daily
Community Health Worker Outreach Activities

Access to Care increase insurance enrollment and limit use of ED for primary care
Obesity  decrease percentage of population considered overweight or obese by 5%
Women’s Health  increase rates of preventative care; decrease teen pregnancy rate; decrease birth outcome disparities
Mental Health  decrease usage of ED for mental health needs; improve RWJF County Health Rankings “poor mental health days”

20. Grants announced include:
$586,735 to Waco Foundation/Prosper Waco
EHF’s grant supports a pilot project that uses Community Health Workers to help at-risk residents become connected to the health system in Waco. The project connects residents who were often going without care to community clinics and doctors. The goal is to improve their access to medical and mental healthcare, adopt healthier lifestyles and manage chronic health conditions.  The project was created after the Prosper Waco initiative asked a group of residents about what change was necessary to help those most in need throughout their community. Along with connecting residents to clinics, the grant helps strengthen clinics’ ability to serve more patients.
