Simplify My Meds is a coordinated prescription refill program. It synchronizes a patient’s prescriptions to be refilled on the same day each month. Goal is to lower resistance for refills and access to medications. Creates the opportunity for an appointment based model.

One Thing Less to Worry About
Less patient calls for prescription refills. Refill requests will be handled by the pharmacy. Less time spent checking the same patient's profile again and again. The pharmacist will review the patient's prescription list each month and will also monitor any changes from the prescriber. Increased patient adherence to therapies. Better tracking of patients with multiple prescribers.

The Patient Advantage The pharmacy will coordinate refills for multiple family members so they are filled on the same day. Patients will receive a reminder call from the pharmacy when a patient's refills are ready. Delivery is available in most areas or to the prescriber’s office. Monthly refills allows for an appointment based model giving the patient more interaction with the pharmacist.

How Does It Work? One week before the patient's scheduled pick-up or delivery, the pharmacy will call the patient. The pharmacy reviews the patient's medications with them, to evaluate adherence and ADRs, answer any questions, and look for possible ways to save the patient money. The pharmacist will review the order and contact the prescriber's office if refills are needed or the patient reports a change in therapy. The patient will receive a reminder call when the refills are ready.

How Do Patients Enroll? Patients can self-enroll at the
pharmacy or can be referred by the prescriber. They bring all of their prescriptions as well as their vitamins or supplements to the appointment. Patient and pharmacy establishes refill date. Patient and the physician receive a summary of the patient's drug list including medications and prescribers.
