Tier 4: HIPAA violation due to willful neglect and not corrected. PENALTY: $50,000 or more per violation, up to an annual maximum of $1.5 million for repeat violations.

PENALTY: $50,000 or more per violation, up to an annual maximum of $1.5 million for repeat violations
