2 Registration Process The registration process is an integral part of the patient experience here at Community Hospital of San Bernardino It is ESSENTIAL that patient’s are registered correctly to ensure That the correct patient receives treatment To ensure each patient’s information is kept confidential That charges are billed to the correct patient

3 Admitting Registration Process
Using the patient’s social security number to register a patient is preferred This method can ensure discrepancies and misspellings are avoided This prevents searching the Master Patient Index (MPI) through many names If a social security number is not available, unknown or if the patient does not have one, you can register the patient using last name comma first name. You would have to allow the system to check the MPI , see the name possibilities before selecting

4 Registration Example Scenario: During the face to face interview conversation with the patient or patient’s representative the Admitting Representative notices some discrepancies with the demographic information that is provided vs the information that is in Meditech Types of discrepancies Misspelling of name Duplication of Medical Record Number Selecting the incorrect patient Demographic errors

5 Demographic Changes Making Demographic changes should trigger you to ask more questions. Most important question being; was the correct patient registered? Demographic discrepancies/changes may not only occur during the registration process in Meditech You may identify demographic discrepancies/changes during the face to face registration using a face sheet for verification For Name, Date of Birth or Social Security Number Discrepancies, ask for the patients Identification Card or social security card for verification.

6 What Type of Identification is Needed at Registration?
Upon full registration two forms of identification and proof of insurance are required

7 Verification of Address
It is not uncommon to for a patient to have an address change The staff member notes the following information in Meditech Previous Address: 123 Medical Dr. San Bernardino CA 92411 Current Address: 456 Center Dr. Colton CA 92324 Scripting: Ms./Mr. Jones, have you previously lived on Medical Drive, San Bernardino? If not, has the correct patient been registered?

8 Verification of Next of Kin
The staff member is conducting verification of Next of Kin and notes the following information in Meditech Previous Next of Kin: Spouse Doe, Peter Current Next of Kin: Spouse Doe, Sam Scripting: Ms. Jones, have you listed anyone other than Doe, Sam in the past? If not, has the correct patient been registered?

9 Patient Visit History Query
Ask the patient if they have been here before Perform a “Patient Visit History” query in Meditech This can be used as a method of verification to ensure you have registered the correct patient NOTE: Always verify patients demographics (per policy: Verification of Demographics ) Confirm demographics before making changes or updates this will ensure that you have selected/registered the correct patient

10 Main Admitting/Community Medical Plaza (CMP)
The Admitting Representative will ask the patient or patient’s representative to review the face sheet for discrepancies The Admitting Representative will ask the patient or patient’s representative to initial the face sheet approving its accuracy

11 Emergency Room Registration
The Admitting Representative will ask the patient or the patient’s representative to verbalize their demographic information Any verbal information given by the patient or patient's representative that is different than what was completed on the patients information sheet upon arrival or on the face sheet will be discussed with the patient or patient’s representative

12 Financial Counseling Financial Counselors that come in contact with a patient or patient’s representative either in person or telephonically for financial screening will ask the patient or patient’s representative to verbalize their demographic information All discrepancies will be verified and corrected by the Financial Counselor
