A, Low power photomicrograph of heavily pigmented melanoma involving entire length of hair follicle to base of the biopsy, including sebaceous duct, with invasion into adjacent papillary dermis. The Breslow depth was measured from inner root sheath of the hair follicle to the deepest penetration into the papillary dermis. There is no involvement of the shoulders of the follicular infundibulum or adjacent epidermis. B, Upper follicle showing markedly atypical melanocytes singly and in small nests occupying the full-thickness of the follicular epithelium with high-level Pagetoid spread through to the inner root sheath with shedding into the follicular lumen consistent with in situ melanoma.
