5 Humana Vision Plan (Wholesale Frame Allowance)
$40 frame allowance = $ retail price Benefits will be the same whether you buy your frames at a participating private practice office or retail chain Members pay 2X the difference between the wholesale cost and the frame allowance. Ex.) $120 retail price = $40 wholesale price $50 - $40 allowance = $10 x 2 = $20 member copay

Hundreds of private practice optometrists. No difference in benefits when using a private practice optometrist or a retail chain as long as the provider is in network. Retail Chains within the network include:

LASIK benefits are available to any enrolled member Members can simply show their ID card to a participating LASIK provider and receive a free consultation The network locations listed below offer the following per eye prices: Conventional/Traditional Custom TLC $ $1,295/$1,895 LasikPlus $695/$1,395 $1,895 QualSight LASIK $895/$1, $1,320/$1,995 You can also use independent LASIK provider network doctors to receive a 10% discount from unusual and customary prices and pay no more than $1,800 for Conventional or $2,300 Custom.
