Members pay 2X the difference between the wholesale cost and the frame allowance.
Ex.) $120 retail price = $40 wholesale price
$50 - $40 allowance = $10 x 2 = $20 member copay

No difference in benefits when using a private practice optometrist or a retail chain as long as the provider is in network.

LASIK benefits are available to any enrolled member
Members can simply show their ID card to a participating LASIK provider and receive a free consultation
The network locations listed below offer the following per eye prices:
Conventional/Traditional Custom

QualSight LASIK $895/$1, $1,320/$1,995
You can also use independent LASIK provider network doctors to receive a 10% discount from unusual and customary prices and pay no more than $1,800 for Conventional or $2,300 Custom.
