2. Identifying Resources in the Community
An important step toward becoming independent is developing an awareness of the resources in your community that are available and specific to your needs. Each person is unique with regards to what resources they will need to use. In this module, we will discuss a variety of topics that represent common concerns. As you work through the module, think about how each topic applies to you and what additional resources you may need to seek out.

3. Health Care
As you get older, you will take on more responsibility in managing your health care needs. Some topics on which you should become familiar include:
Your medical diagnoses and prescribed medications
Your health care insurance(s)
The contact information for your doctors and whom to contact based on symptoms
Health care locations on and off the university campus

4. Health Care
You will also need to transition health care services from your pediatrician to a primary care physician (PCP).
If you would like your parents to have open communication with your PCP, you will need to complete a HIPPAA form at the doctor’s office.

5. Mental Health Care
In addition to your physical health care needs, taking care of your mental health is equally as important.
Reach out when you need support! Become familiar with the counseling and support group services available to you both at the university and in your community.

6. Transportation
Up to this point, you may have relied on your parents to drive you places or you may have taken the school bus. You will need to consider what mode(s) of transportation will be most accessible for you. Some options may include:
Drive your own car with or without adaptive driving technology
Ride a bicycle
Car pool with a friend
Use paratransit or the local bus
Take the tram

7. Housing
You will need to determine the housing arrangement that will be most accessible and appropriate for your situation and individual needs.
As you decide on the best housing option for you, here are a few things to consider:
Speak with your parents to discuss what housing options you can afford. If you receive financial aid, contact the financial aid office at your university to inquire if loans/scholarships can be applied to housing expenses.
Think about your preference to live alone or to live with a roommate.
Consider potential housing accommodations that you may need.

8. Resources in the Community
Connecting with resources in the community will also be important as you transition out of high school. Some resources that may be of interest to you include:
