1. Parent Initiated Withdrawals
Please first attempt a win-back
When a parent requests a WD you will fill out the WD by clicking on the AZVA Teacher K5 Parent Initiated link- on next slide 
You will also send the parent the link to the legal guardian to fill out on their side as well.
Once you submit the WD it goes to Sheri to approve and will then go to the registrar to process. 
If anyone other than the Homeroom teacher submits a withdrawal (FAST, Blended learning, etc.) you as the HR teacher will receive a request to approve the WD before it processes.  We wanted the HR teacher to have the option of doing a win-back call prior to the WD processing and we didn’t want any surprises either and you not knowing that one of your students have been withdrawn.
