4 Outcome 1: Children, Youth & Adults are Physically and Mentally Healthy
Priority Services and Programs for Children, Youth and Adults Implement With Every Heartbeat is Life Program – Urban Strategies will train residents to educate their neighbors on how to live healthy lifestyles and raise awareness of the risk factors for heart disease. Increase access for adults to the Tulane Health Center – Neighborhood based Federally Qualified Health Clinic that is nationally recognized for their community based health services around both mental and physical health. Increase access for children and youth to the Louisiana State University Tiger Care Pediatric Clinic – Neighborhood based Federally Qualified Health Clinic that specializes in the mental and physical health needs of children and youth. Will implement their new obesity screening program. Increase access to the New Orleans Food for Families Program – Provides nutritional food to at risk populations such as pregnant women, seniors, and parents with children under the age of 6.

5 Outcome 2: Children Enter Kindergarten Ready to Learn
Priority Services and Programs for all Iberville Children Engage The Early Childhood & Family Learning Foundation (ECFLF) – 1) Provide technical assistance to neighborhood child care organizations; 2) Assist K-12 schools in implementing Coordinated School Health Programming that includes nutrition education and physical education programming; 3) Partner with Urban Strategies case management to link families to needed services for children; and 4) Develop a plan to place its second Early Childhood School in the neighborhood which will increase childcare slots. Enroll children into Total Community Action’s (TCA) Early/Head Start Program – Full-day child care program with a 1:4 child to caregiver ratio. TCA will provide parental involvement opportunities that include volunteering in the classroom and participating on the Parent Committee. TCA expects to expand the number of available child care slots in the next year.

6 Outcome 3: Children are Proficient in Core Academic Subjects
Priority Services and Programs for Children, Youth and Adults RSD Reform Plan – 1) Develop teacher and school leader effectiveness; 2) Implement comprehensive instructional reform strategies; 3) extend learning and teacher planning time and create community-oriented schools; and, 4) provide operating flexibility and sustained support. Urban Strategies Community Education Specialist – Hire a Specialist to support neighborhood parents, neighborhood schools and the Iberville/Treme Promise Neighborhood Inititiave.

7 Outcome 4: Youth Graduate from High School College and Career Ready
Priority Services and Programs for Children, Youth and Adults Work in partnership with the Youth Leadership Board and the Recovery School District to increase access to the following program partners: Youth Empowerment Project – will provide holistic, wrap around services to at-risk youth The Partnership for Youth Development– will leverage resources to programs in New Orleans that serve youth during out of schools hours Urban Strategies its school partners commit to identifying additional high quality programs for youth around academic achievement, recreation, financial education and life skills

8 Outcome 5: Households are Economically Stable & Self-Sufficient
Priority Services and Programs for all Iberville Youth and Adults Industry Based Business & Employment Training – Place based hard skills training and job placement targeting the following sectors: construction, including building crafts & trades, healthcare, culture, culinary arts, hospitality and tourism, oil & gas, child care, and transportation. Focus on construction to support the Transformation Plan and numerous physical revitalization projects throughout the City. Literacy and GED Programming – Lack of at least 6th grade math/reading proficiency has often prevented HANO subsidized adults from even entering job skills training. The YMCA will provide a basic adult education program including math and reading education, and the Youth Empowerment Project (YEP) will provide GED preparation & testing services. Job Readiness Skills – 21st Century Success Principles, a nationally recognized curriculum, to prepare residents for gainful employment Partners: YMCA, Goodwill Services, and the Sojourner Truth Center. Integrate Programs with Youth Services - Urban will develop a Youth Leadership Board (YLB) to engage youth in charting their development opportunities. The YLB will focus on developing neighborhood youth into leaders within their community. The YLB will focus on multiple topics including the Plan, education, employment & business development, safety, and other issues critical to youth development.

9 Outcome 6: Residents Feel Safe in Their Neighborhoods
Priority Services and Programs for all Iberville Families Coordinate with the Mayor of New Orleans Empowerment Task Force – Develop effective crime prevention strategies in partnership with the New Orleans Police Department, HANO, the faith based community, local businesses and community residents. Implement a Neighborhood Watch Program – Leverage residents’ confidence in HANO’s security to organize engaged resident leaders through recognized community policing strategies. Engage resident youth – The Youth Leadership Board will actively work with local partners, including law enforcement, to develop crime prevention strategies and engage youth in positive after school activities. Integrate physical design principles that support a safe neighborhood – Integrate best practices around urban design that support a safer community, such as improved lighting and reintegration of Iberville into the surrounding neighborhoods. Public Safety Enhancement Initiative focused on bringing real economic opportunities to resident.

Critical partnership with the Iberville Resident Working Team Support for resident entrepreneurs and leaders Case Management launched in the fall of 2011 Data and Outcomes Driven Service Coordination Case Management linked to all People Services and Strategies Staffing Plan (qualified Iberville residents will be a priority for filling these positions) One Project Manager One Supervising Case Manager Four Case Managers Three MSW Interns One Education Specialist Two Employment Placement Specialists Two Outreach Workers
