4. Initial Review Kate will tell a Reviewer Reviewer checks notes
Interview with someone who knew the person well
May be a multi agency review
Will need to have a clear case note request process in place – contact details, expected timescales for return etc
Local areas will need to consider how they will manage conflicts of interest – eg, if reviewer has had involvement with person – and how long ago
Identify one set of case notes based on where person died
Interview someone who knew the person well – family member, friend, support worker etc. Purpose is to develop the richness of detail about the events leading upto persons death and perceptions of treatment
