3. Why Review our Service
To ensure that adults with a disability who have been assessed as requiring day services have their needs met either within a day centre or day opportunities in local communities
To address current capacity issues within day care now and for future provision
To address current and future staffing requirements
To ensure appropriate facilities are provided which are fit for purpose and meet service users needs
To determine most appropriate service model for current and future service delivery

9. Reporting Mechanisms
Sub-group leads will report back to the Project Manager on a monthly basis

Project Manager will provide monthly reports to the Project Owner on a monthly basis addressing

Communication issues
Proposals for Action for the next 4 weeks

10. The Review will conclude by September 2018 with recommendations to SMT/Trust Board for the way forward
