1. What is Partners in+Care? Partners in+care is for you as a patient. For you to be more proactive in staying in care and recruiting your peers to get back into care. This includes resources for you and support along the way. 2. Who can join Partners in+care? People living with HIV, and their friends, family, or support system that help them stay healthy. It also includes social workers, case managers, and peer educators, and others involved in educating patients about HIV and HIV care. 3. Will my information be confidential? Yes. There’s an option to remain anonymous if you’d like. You can share as much or as little information about yourself as you’re comfortable. 4. Where I can learn more?
