How should Connecting Communities teams evaluate and prioritize the implementation of patient-facing EHR or PHR application as they roll out their program? What lessons have been learned from early PHR projects that can help Connecting Communities teams plan for their own work? How much should Connecting Communities projects work together or conform to national standards vs. just solve local problems as local interests see fit? How can features of the EHR roll-out be designed to maximize benefit to and use by patients and families?
