1. FAFSA Verification Requirements
Please do not complete these forms until you have been instructed to do so.
If you are selected for Verification, you must provide the following to your Financial Aid Officer:
Verification Worksheet – Please complete, sign and date
IRS Tax Return Transcript
1099’s (if applicable)
W2’s for student (and spouse/parent if applicable)
Any other forms that your Financial Aid Officer has requested to complete or submit
NOTE: If documents are not received within 15 business days from the time you were notified, you may not be eligible to receive Federal Student Aid for that award year.
