What is a Continuum of Care?
A Continuum of Care (CoC) is a program designed to promote a community-wide commitment to ending homelessness. We do this by: Providing funding for non-profit contributors, States and local governments to quickly re-house the homeless and victims of domestic abuse Promoting access to mainstream programs for the homeless Optimizing self-sufficiency among the people we serve

Coordinated Entry Coordinated Entry is a powerful tool designed to ensure that homeless persons and persons at risk of homelessness are matched, as quickly as possible, with the intervention that will most efficiently and effectively end their homelessness.
