Situation Statement: Goal: Target Population: Equity Considerations: Assumptions: External Factors: Knowledge Synthesis Input Activities Output Outcomes Short-term Medium-term Long-term -Needs assessment -Gap Analysis -Lit Review -Environmental Scan -Staff -Space -Time -Dollars -Equipment -Expertise -Outreach -Communication activities -KTE -Other -Flyers -Workshops -Webinars -Modules -Posters -Other Change in: -Knowledge -Awareness -Perception -Understanding Change in: -Perceived belief -Attitude -Behaviour -Practice Impact: -Improved access to care -Improved quality of life Situation Statement: The situational context that lead to the creation of the program, i.e. ‘current state’ Goal: The overall long-term health outcomes the program/initiative hopes to achieve. Target Population: Who is the main population your program/initiative trying to reach? Equity Considerations: How does your program affect health equity for the identified vulnerable or marginalized populations? What are their determinants of health considerations? Assumptions: Underlying theories and beliefs about the program and its context which can influence the development of a program and which activities are implemented. Transparency around assumptions makes explicit the beliefs that underlie chosen actions. External Factors: Factors that impact the program but are beyond the control of program planners/overseers.
