Background Banner Mobility Assessment Tool (BMAT)
CMC tool of choice to assess the patient’s balance and strength Used to determine the equipment necessary for safe patient handling Historically, bedrest orders were initiated upon admission and not re-visited, bedrest orders were written and not followed, activity orders written and not followed, assumptions that therapy set activity orders As a result, patients suffered deconditioning Prolonged length of stay Inability to return home (sent to SNF instead) Risk of HAPU, HAP, DVT, PE, and ileus Increased risk of falls

Major changes: Using past tense and language with an order as opposed actual event happening Using language consistent with early mobility and assistive devices

Major Changes: Changed row from “repositioned” to “positioned” Current terminology for assistive devices

Major Changes: Terminology consistent with safe patient handling equipment

Pre-Mobility Assessment - NEW
Pre-Mobility assessment done before BMAT to determine patient’s ability to participate in early mobility To be done in conjunction with admission ADL screening

BMAT Assessment – NEW BMAT assessment will determine the level of activity and the assistive device needed to mobilize the patient to the maximum of their ability NOTE: Extensive education will be provided on use of assessment tool

Worklist – Activity Documentation

Orders – Bed Rest New orders for bedrest will guide the ordering provider Facility approved reasons for patient’s non-participation in early mobility If patient doesn’t have any of these conditions, provider can consider early mobility

Orders - Bedrest This order will require the order provider to select why the patient cannot participate in early mobility

Orders – Bedrest then Mobility
This order will allow the provider to instruct the nurse to keep the patient on bedrest for 12 or 24 hours, then progress the patient to early mobility

Orders - Mobility This order is for the nursing staff to move the patient according to his/her ability

Worklist – BMAT Assessment
A worklist task will trigger the nursing staff to mobilize the patient at least twice a day
