MCOSA requires treatment providers to offer clients timely access to an initial assessment as well as ongoing services. For Performance Indicator report purposes, any Initial or Ongoing appointment that did not occur within 14 days of request, must be have a justification provided within the Focus system. Initial Appointment information is recorded in the box below, found in the Admission form. Date of First Request/Contact – date the client or Access Center first contacted agency for services. Admission Date – date client entered treatment/assessment date. Time to Treatment –this will automatically calculate based on the Date of First Request and Admission Date – the number of days needs to be 14 or less to be timely. If it is over 14 days, Reason admission is outside of 14 days of first contact will appear and needs completed. If the reason time waiting is more than 14 days due to client requesting a later appointment date, mark the check box and add comments explaining first available date the program could have offered in the comment box or the reason for client’s request to wait for services.

Client’s “On-going” (second) service is required to be within 14 days of the Initial/Assessment appointment to be considered timely for Performance Indicator reporting. This is reflected on the bottom of the Admission screen in Focus. Select the “lookup”botton and add your program name. Enter the date of the ongoing appointment you offered to the client in the ‘Date Offered’ box. If the client accepts this appointment, enter the same date in the ‘Date Accepted’ box. If not, indicate the next available appointment date the client accepted. If the client requests the On-going service to be more than 14 days after admission, check the “Consumer requested an appt outside 14 days…” box and provide the reason in the comment box below. If a client chooses/does not to return for treatment after the assessment, select the “consumer refused ongoing services” box.

If the Initial or ongoing service did not occur within 14 days of request/admission due to a program related issue, enter the reason in the comment box for that type of service. Note: the program is in violation of the MCOSA contract when this occurs and should notify MCOSA if there are issues with timely services If the client does not keep the timely Initial or Ongoing service appointment scheduled by the program, complete a Twin 14 Form. This form allows the program to show it had timely services available and provide the reason for the exception to the timeliness rules. See the following slides for details.

Enter date this form is being completed. Select “type” of appointment you are providing information for: 1)Initial = Assessment Appointment, or 2) Ongoing = Second Appointment. Only one appointment type can be used per form. If both the Assessment and Ongoing appointments were beyond 14 days, a separate form would be filled out for each.

Complete the information in the right side column, beginning with “Consumer did NOT attend …” Select the “Reason for not attending”, then a choice from Was appointment rescheduled”. “Save” document.

You are then returned to this screen, where you can select “Add Twin 14” again, if a screen is needed for the Ongoing (2nd appointment). This area provides you with the ability to change or view the completed form as necessary.

Required For Detox Discharges: Check the box below on the Discharge form in Focus if the client refused services after discharging from detox. This should be checked if a client leaves early/against advice and does not schedule the next level of care appointment. If a client requests services more than 7 days after detox discharge, complete the “Consumer requested an appointment outside of 7 days…” box Complete a Twin 14 Form for Detox if a client declines an appointment at the next level of care within 7 days from discharging from detox and fill out all the questions in the right side column.
