2 Enrollment At the end of April, we had 160 funded slots filled.
100% Enrolled 160 children Attendance (ADA) 89% - March 2017 89.79% - April 2017 Agency Wide: 93% of children meet Federal poverty guidelines, received public assistance or are in foster care/homeless 9% (15) on Waitlist 19% (30) of enrolled children have special needs At the end of April, we had 160 funded slots filled. Our attendance for the month is 89.79% C If under 85%, we had to provide an analysis report. 93% of families are at or below the federal poverty levels, receive public assistance or are in foster care. C+2004 9% on Waitlist. C+ 2006 19% (30) of the children being served have been diagnosed with special needs. C (Total Diagnosed)

3 Health Health Requirements 95.6% (174) Dental Screenings Need Dental Treatment 9.7% (17) Receiving Dental Treatment 100% Mental Health 16 95.6% (174) of the children have up to date Immunization and complete physical exam. C (c11(2)/total) 95.6% (174) of the children have completed dental screenings for children. C (c19/total) Out of 174 dental screenings, 17 needs dent treatment. Medical Homes 100% Dental Homes Insurance Performance Standards required that the agency help families secure a consistent source of medical and dental services as well as medical insurance. Currently, 100% (182) of our families report having a medical home. C (c5(2)/total) 100% of families with a single source of dental care. C (c17(2)/total) 100% of families with Medical Insurance.
