INCIDENT
Electrical team along with painter was rigging down electrical cables in preparation to
move. During the operation the painter lost his footing, slipped and his right knee struck
the cable tray resulting in a undisplaced fracture on the right patella.
ROOT CAUSES:
Lack of Due Care and Attention: Each employee is responsible for his/her own safety
Inadequate Supervision: Inadequate identification of loss exposure
Improper Motivation: Worker was on his last day of a very long hitch
Lateral Learning
Everyone is accountable for their own safety
Simple routine tasks should not be underestimated.
Stand back 5x5 for all conditions, think before you act.
Keep your work place tidy and watch out your step
Recommended Actions:
Discuss this alert in toolbox talks and HSE meetings and
Distribute to all contractors
Post on HSE notice boards
Include in site HSE induction
