Introducing John Better Futures Baseline Assessment Header: Accommodation Avg 4 1.1 Suitability of Property 1.2 Security of Accommodation Header: Health Avg 1.5 2.2 Mental Health 2.3 Addictions Header: Safety and Security Avg 4 3.2 Legal Issues Header: Social and Economic Wellbeing Avg 3.7 4.1 Life Skills 4.2 Money Matters and Personal Administration 4.3 Leisure 4.5 Self Esteem John was welcomed into QMD Aspire on 19th September 2014. He presented at the service with multiple complex needs ranging from Addiction, Homelessness and Criminal Justice.. This is a representation of John’s Batter Futures Baseline Assessment, for those who don’t know about BF:- 4 = Crisis 0 = No support Needs

How different could it have been for John?
John was given the chance to reflect on how his journey may have progressed without the support he has received. 1.1 John believes he would have ended up skippering and ultimately rough sleeping. 1.2 John scored this with the thought of not having accommodation to sustain and due to addiction being unable to sustain any accommodation he was offered. 2.2 John believes his mental health would have deteriorated and that he would be unable to address this. 2.3 he feels his addiction would have spiralled out of control with it becoming his single focus. 3.2 john has scored this as though he had ended up having to deal to maintain his addiction therefore having a high likely hood of a custodial sentence at some point. 4.1 he has scored this as crisis not because he is unable or does not have the skills simply that the addiction would stop him using or prioritising them. 4.2 linked again to the addiction serious financial problems would have followed. 4.3 as above John would simply not be motivated to take part in any leisure activities. 4.5 all of the above would in Johns opinion have left him with very poor self esteem and a lack value in his worth.

“Looking at the visual graphs focuses me on wanting to achieve the best score possible and achieve the goals I am aiming for” “I knew where I had been in the past, this allowed me to identify areas where I knew I wanted to and needed to achieve to give me the life that I feel I deserve” What was the best thing about Better Futures for you? How did Better Futures help you to identify your developmental support needs?

Staff views on Better Futures
“It can be very positive for an individual to visually see their own progress and even if it looks negative this can still have a positive affect on individuals and their motivation” What as a staff member is the best thing about Better Futures for you? “It helps support individuals to identify their own support needs. It also generates the discussion over an individuals outcomes and negotiation over where an individual is on their journey and how they are going to achieve their outcomes and aspirations” How does Better Futures help you work with an individual to meet their developmental support needs? “It gives a guideline over what developmental support is required for an individual and allows the plan to be tailored to meet their individual needs. At the point of review the individual and staff member are both able to see how the outcomes have been achieved”
