Utilizing the information that is provided on the DA Form 1156, Casualty Feeder Card, complete all applicable fields on the Hero Card.
The following additional information is provided:
The casualty is 25 years old and is from Dallas, TX. He was an 88M based at Fort Lewis, WA.
Remains are expected to be transported to CONUS o/a Family will be presented with the Purple Heart during the funeral services.
The Soldier’s religious preference is Lutheran.
