Coordinated Entry Flowchart Weekday
Client comes to shelter on weekday Client or Staff fills out Agency intake packet Client signs HMIS release HMIS release is scanned and sent to CES via Yammer Client is connected with CES via phone This should happen within 24 hours of client arriving at shelter-Per HMIS Policy and Procedure CES completes VI-SPDAT CES enters client into Agency HMIS

Client comes to shelter after 5 pm or on weekend Client meets with RA Staff and client fill out intake packet Client signs HMIS release All documents are given to Case manager so that they can be sent to CES via Yammer Client after 5 on weekday Case manager meets with client the following day HMIS release is scanned and sent to CES via Yammer Client is connected by phone to CES to complete VI-SPDAT Client comes to shelter on weekend leaves before Case manager sees them Case manager sends intake information to CES via Yammer so that client can be entered into agency HMIS for case manager to make case note and exit
