When you enter your “My Profile” account, scroll to the bottom of the page and you will see a box that says “Create New CCP Worksheet”
Click on “Create New CCP Worksheet”

5. At the top of the page you see 4 tabs
At the top of the page you see 4 tabs. You must complete the “Worksheet Details” before you can move on
Remember to hit save or Save and Continue

6. You can now proceed to the self assessment

Save and continue allows you to move on to the next standard

8. As you move through each tab you will see the tile change to black

9. In the column “I will focus on this indicator this year”, place a check in the box of the indicators you will work on this year

11. You can now save the assessment and move on to the learning plan

12. Learning Plan: You will see the indicators that you have specified that you will work on. At this point you will identify and enter your learning objectives

13. The next section of the learning plan is to identify a learning activity to meet you learning objective. Once finished, save and continue
