The Personalised Transition Plan

Where its used During year 9 (13-14 years old) all statemented students are given a transistion review As well as discussing progress and needs it is also the chance to talk about future plans Due to this it is a statutory requirement for the Connexions PA to attend

Where its used The review takes the form of a meeting bringing together student, parents, teachers, connexions PA and any other involved professionals such as physiotherapists, and social workers The young person is expected to tell us what they are wanting to do in the future etc in the meeting – which is often intimidating

Why use the plan? It puts the young person at the centre of the planning process Young people often feel too intimidated to talk in front of so many adults so the plan is used to give a voice to the young person. Parents can feel the same and is some circumstances the plan is also completed with the parents

Filling the Plan In Different schools use the plan in a different way In some cases the PA and a member of staff from adult support services home visit the client and the parent Another way is to work through them in a group session and keeping them as an ongoing project

Why is it so good? It can be completed in a number of different ways including cutting out pictures and sticking them on etc It gives the young people the chance to say what is important to them – not just what the professionals thing is important to them

It might be as random as an interest in trains

What do we do with the information? The plan is brought to the review Things can be added to it as it is a constantly evolving document Parents and professionals use the information to look at future opportunities for the young person this can include post 16 options or just hobbies and interests that they wish to persue.

What difference has it made? Young people are given the chance to talk about what they want to do, this has brought up desires that the young person has never mentioned before – even if this is as simple as wanting to go to a dance class it can it can enhance their quality of life

What difference has it made? In the past young people havent been brave enough to speak about what they want to do and this has sometimes resulted in young people not enjoying courses / placements dropping off and becoming NEET. Now by getting more involved they have more input and are thus more likely to get something that interests them and they are more likely to stay on the course / placement

What difference has it made? Young people feel a sense of ownership surrounding the transition plan. They take a more active part in the process giving them a better sense of involvement and importance They gain more enjoyment out of the process and take a pride in the work that they complete
