Refreshing our Priorities

In 2014, we agreed our priorities for the next two years (known as our transformational changes).
At that time we also agreed our outcome ambitions, some stretching to 2019.

Progress so far Integrated Community Locality Teams - Developed working in partnership across Sunderland, model agreed in principle, Looking to mobilise over the next 12 months Care Homes – Pilot in Coalfields producing very good outcomes, Looking to roll out across the city as part of Integrated Community Locality Teams Intermediate Care Hub – Hub soon to be operating from Leechmere now operating extending hours and additional beds available at Farnborough Court moved towards 24 hour single point of access by September End of Life deciding right - Care Home and GP training underway. Mental Health – 5 year programme to develop a model of care for Sunderland is now near the end: Enhanced IAPT services, New model of psychological therapy, new hospital environments at Ryhope & Monkwearmouth, Ongoing implementation of improved community mental health services.

Progress so far Urgent Care – GP urgent care centres are now operational, GP Out of hours procurement is currently underway aswell as the development of City Hospitals emergency department urgent care centre. MSK – The procurement of the new MSK service is complete and the new provider will be announced shortly. Dementia – A dementia friendly community pilot is running well in Houghton. All staff in GP practices across Sunderland will be trained on Dementia Awareness by end of March The Essence service is now in place and receiving referrals. Procedures of Limited Clinical Value - Phase One of the value based commissioning policy was implemented in January 2015, with full implementation by April 2015.

Moving forward into 2015/16 we have undertaken a light touch review of our priorities considering the progress we have made so far along with national guidance. We will still be continuing all of the priorities we identified in 2014, however we have also identified some additional priorities. Some of which are new and others are things that we are already doing which we feel are significant enough to highlight them on our plan.

Proposed new priorities
Influence a prevention & self management approach Develop a strategy with Sunderland Council to improve outcomes for children Develop and implement a strategy for General Practice Implement transforming lives for people with learning disabilities Implement the new model of care for people needing continuing healthcare Prevention & Self Care Programme will be developed with Public Health and other key partners Children services – Currently working with the local authority to identify what our role will be in improving outcomes and what will be the focus for this work General Practice Strategy – we will work with our member practices and other partners to develop a plan for primary care Learning disabilities – Focus on improving outcomes for people with learning disabilities Continuing healthcare - Free care outside of hospital which is arranged and funded by the NHS. This work is already underway and we are continuing to work with the local authority to implement an improved model.

Outcome Ambitions Proposing to maintain existing ambition with exception of: 5 year trajectory for Potential years of life lost achieved (PYLL) – Looking to apply further stretch As part of the refresh we are revisiting our outcome ambitions and identifying those which we may need to revise due to our performance. In 2014 we proposed an improvement of 7% for the potential years of life lost for people in Sunderland. This measure is the estimate of the average years a person would have lived if he or she had not died prematurely. It is, therefore, a measure of premature mortality. Public Health colleagues are currently working on a revised proposal.
