2 BACKGROUND Over the last 15 years prevention has become core business for FRS Impact on Fire & Rescue Service’ response activity Based on analysis of incident data and further research, the risk factors that increase vulnerability to fire are aligned to those that may affect health & wellbeing

Further response capabilities to health related emergencies (EFR schemes / Falls response) Working together in prevention (including data/intel) Targeted Home Safety Visits to vulnerable people from fire to Safe and Well Visits Supporting health and wellbeing agendas across the community (evolving activities)

4 Hull - Falls Intervention Response Safety Team (FIRST)
Background Identified by health that there were a high number of falls in Hull A more rapid response could prevent complications caused by long lie e.g. ulceration and consequently relieve pressure on health. HFRS, Hull CCG and other health partners agreed to provide a pilot response service using operational firefighters in October 2015. Intervention The team attend non injury falls within the Hull area and provide a range of medical observations, assisting in indicating levels of risk to falls –referrals are generate to OT teams. Safe and Well assessments are completed, including low level assessments. Aids and adaptations provided.

5 Hull - Falls Intervention Response Safety Team (FIRST)
Delivery Hull FIRST is made up of 10 firefighters based at an operational fire station in Hull – the team work alongside operational firefighters who offer assistance where required within the referral process Average response time of 25 minutes (agreed max. 60mins) Calls are received through a number of pathways including YAS, CHCP, Lifeline and Care Homes Attended an average of 30/month in year one and now almost 150/month. Future Continue to improve delivery to vulnerable people Move upstream – prior to first fall (data?) Roll out wider if demand is there

Aims: To reduce the demands on services through commissioned pilot Falls Intervention delivered by HFRS Advice and information Onward referral Low level interventions Overall objective: To reduce the numbers of people aged over 65 who fall

10 Integrated Care Centre - Hull
PREVENTION Integrated Care Centre - Hull Hull CCG have been is working with partners including City Health Care Partnership, Hull City Council and Humberside Fire and Rescue to build a new Integrated Care Centre (ICC) in Hull. The ICC will provide tailored care for the vulnerable elderly to prevent unnecessary hospital admissions. East Hull fire station will be relocated in June 2018 to the new facility. Station personnel will support the rehabilitation and recovery of patients following a period in hospital. The project will improve health by helping different public services, including health services, the council and emergency services, to work closely together.
