(A) After completion of intracardiac repair, the left superior vena cava is divided at its entrance into the roof of the left-sided atrium. The hemiazygos vein is also divided. A trapdoor incision is made in the medial aspect of the right superior vena cava in such a fashion that an anteriorly hinged flap is created.
