2 What is Supported Living?
A living environment with an element of shared core day or night support available to meet the needs of the tenants A cluster of single occupancy units which are grouped together either within a purpose-built block or within a defined area (such as a street) or a shared house or bungalow where tenants have their own bedroom (ideally with an en suite) but share every other part of the property with their co-tenants. Accommodation should, at a minimum have a fully equipped kitchen and a communal living/dining area.  All properties are tenancy based, with the landlord separate from the support provider.

Ensure that all adults have an assessment that identifies needs under the Care Act 2014 Use a person centred approach; working with the adult and their support networks to identify assets and strengths informing an assessment of eligible needs and enabling independence Individually review adults at the same scheme at the same time, considering compatibility Consider whether support can be provided more effectively through a core model of delivery Consider needs which may require 1:1 support Ensure that services commissioned to provide support represent best value

Stage 1, 2 and 3 –Scoping and planning Gathering information on the scheme Gathering information on the adults living at the scheme Identifying the tasks required for the scheme review Identifying the scheme lead Prioritising schemes to be reviewed Engaging with provider to ensure they are fully informed of process Identify the social workers who will be undertaking the scheme based reviews Plan the Scheme Review approach

Stage 4 – Completing the Scheme Review Person centered and collaborative Information gathering Evidence based re-assessments Plotting individual needs across the scheme Informs proposals for potential re-modelling of support within a scheme if required

Stage 5 & 6–Proposals, Validation and Sourcing Meet with provider to ascertain, what needs might we have missed and what risks cannot be managed through the proposed model of support Make any necessary changes to the proposed model of support Follow same process and share proposals with adult/representative Hourly rates confirmed with Essex County Council Commercial Team Obtain agreement for funding for support to be sourced Service placement team source and commission the service

Stage 7 and 8– End of Scheme and follow up Reviews Scheme Profile Capture the information gathered on the scheme for future use and reference Follow up Reviews Complete follow up reviews to ensure the revised model of support is working
