Greet the member/new member.
Ask an opening question relevant to the purpose of the meeting, intended to open dialogue rather than accuse of an offense. (ex) Tell me about ______.
Once the member has had an opportunity to express her feelings, review the items checked under “purpose of conference.”
Use CIMS cheat sheet to cover required information.
Record information discussed on report form.
All participants must sign the conference notes. (If the member refuses to sign, it should be noted on the form.)
