Is the carer’s physical or mental health deteriorating or are they unable to achieve key outcomes? Is there or is there likely to be a significant impact on the carer’s wellbeing? Support provided Carer’s Personal Budget Carer’s Personal Health Budget/ NHS Short Breaks – to enable the carer to take a break from caring Access to care and support for the cared for – may include respite or short breaks (may be a charge to the cared-for)

Principles: Quick access to the services that carers tell us they need Personalised support Integrated approach – health and social care services Evidence of outcomes Value for money September 2017 to March 2018 Develop new approach: carer’s personal budgets personal health budgets/ NHS short breaks April 2018 to December 2018 Review and re-commission information, advice and support services for carers November -December: Public consultation – ends 5th Dec November-March: Set up carer working groups Consult with carer networks Develop proposals Council Committee decisions Update Council systems and train staff March-June: Analyse carer population and needs Review all commissioned services – current provision and future requirements (include information, advice, dementia support, young carers) July-December: Produce specifications for enhanced information, advice and support services and tender for new provision
