Pain and symptom management
Assistance with goals of care, advance care planning
Hospice referrals
Caregiver support
F/U inpatient consults

4. How do I refer a patient? REFERAL REQUIRED:
We provide powerchart, or telephone recs to referring provider
We do not write scripts (few exceptions)
Ad-Hoc
I have discussed a palliative care referral with my patient
If referred from inpatient, the outpatient provider managing their care is … xxx
All referrals are screened prior to scheduling (no chronic pain, addiction management)
