Ohio is divided into 12 Area Agencies on Aging Private, non-profit agency Serving a nine-county area responding to the needs of older adults AAAs are the primary vehicles for organizing, coordinating, and providing community based services and opportunities for older adults The Area Agencies on Aging were established nationally with the Older Americans Act in 1965. There are 12 Area Agencies on Aging in the State of Ohio managed and funded primarily through the Ohio Department of Aging. Our region covers in total nine counties, however our office case manages only Montgomery, Clark and Greene

Information & Assistance Ohio’s Aging and Disability Resource Network Assessment & PreAdmission Review In Home Assistance PASSPORT ComCare Assisted Living MyCare They types of services the AAA provides can be categorized primarily in three categories

4 Aging & Disability Resource Network
Made up of twelve AAAs, ADRCs, and statewide single entry points Serve as network of front door to long term services and supports Ohio Benefits Long-Term Services and Supports (OBLTSS) “Answers on Aging” Schedule in-home assessment Provide information and assistance by phone Refer to appropriate agency or website Provide information through mail, , fax In October 2017, the Ohio Department of Medicaid and the Ohio Department of Aging designated all 12 AAAs as the lead entry points into the State of Ohio’s Aging and Disability Resource Network. As a network, combined of AAAs, SEPs, and CMAs, we are designated to serve as a network to provide assistance in linking individuals of any age and need to long term services and supports. We have long been a resource in Montgomery county for Answers on Aging. In 2010 the Montgomery County Frail Elderly Task Force recommended one number for seniors to access information and funded the initial Answers on Aging campaign in Montgomery County. We saw a marked increase in calls as a result and the campaign continues to be funded now by AAA dollars.

5 Assessment & PreAdmission Review
Comprehensive in home assessments completed by licensed Registered Nurse or Social Worker to link to long term community based options. Level of care assessment Financial assessment Medical systems review and medication reconciliation Review options Pre Admission Review is required by the State of Ohio prior to admission to a nursing home that accepts Medicaid payment One service which makes the AAA unique to other service providers is our ability to provide an in person comprehensive assessment once you call.

6 In Home Assistance Programs that Help at Home
PASSPORT Home and community based option An alternative for nursing home placement Assisted Living For those who need more than care at home, nursing home admission is no longer their only option Available in only certified facilities Mycare For those who are dual eligible, Medicare and Medicaid ComCare For those who do not qualify for PASSPORT but need help to stay at home Funded by Montgomery County HSL If determined eligible, our Assessment team can initiate any of our home care programs

Personal Care and Homemaking Caregiver Respite Home-delivered meals Emergency response systems Adult day care Transportation

8 What we do for our consumers Supporting the prevention, reduction & resolution to elder abuse, neglect & exploitation Care Management Creation of individualized care plans including purchased services and community resources Identification and coordination of services and supports to prevent or postpone institutionalization and avoid unnecessary hospitalizations Critical safeguard in assuring services are delivered Ongoing ReAssessment Evaluations to assess changes in health status at home and in nursing facilities, with the goal to keep consumers at home After my brief overview of our Agency, we were asked to share what our role is in supporting the prevention, reduction and resolution to elder abuse, neglect and exploitation. What we do is provide many strategies which involve the same endeavors we are all trying to combat as a community.

9 What we do for our consumers Supporting the prevention, reduction & resolution to elder abuse, neglect & exploitation Reconciliation Reconciliation of daily authorized services against provider monthly invoices Care Transitions Personal coaching and coordination for seamless transitions from institutions to in-home or community based services Evidence based interventions Caregiver Support Information and respite services provided to assist informal, family caregivers

10 Community Collaboration What works & Challenges
Robust resource database of community providers and services In home services are delivered through a network of over 150 providers & 28 assisted living providers in Montgomery County serving our frail seniors Care Managers can link consumers to an array of community supports & providers in addition to the in home services We were also asked to identify and describe any strengths and challenges in our current community approach to preventing, reducing or resolving elder abuse, neglect and exploitation. What works is everyone here.

Must be accepted by the senior or caregiver Must be understood to provide home and community based care Work best when additional supports are present Challenges to optimize a senior’s quality of life and not only prevent but sometimes even the identify elder abuse, neglect and exploitation we find when trying to link elders to the Area Agency on Aging programs and services include: 1. The services must be accepted by the senior or caregiver after education, options, and agreement are established 2. The services must be understood to provide home-based care only and not always every identified need. A common misconception is our resources include more than home based services when really our services are only a part of that care plan. 3. Work best when supports are present such as a. Caregivers to provide support b. External programming to address medical or mental health needs c. Community resources to assist with home repair, safety, or utilities Something I am excited to see as a development out of this work is a more comprehensive referral plan at the front door to enlist assorted community resources that address multiple issues at the onset of an identified need. No singular Agency can tackle more complex, multi-dimensional needs. A stronger safety net woven of more than one community resource can help Montgomery County seniors remain in the community longer and with optimal safety. We are stronger together.
